Washington Consumer Health Data Privacy Policy

This Washington Consumer Health Data Privacy Policy (“Health Privacy Policy”) is provided pursuant to the Washington My Health My Data Act and supplements DwellSafe’s Terms of Use and Privacy Policy. This Health Privacy Policy only applies to our collection of Consumer Health Data (defined below) of Washington State residents.
Collection of Consumer Health Data
“Consumer Health Data” means personal information that is linked or reasonably linkable to a consumer and that identifies the consumer's past, present, or future physical or mental health status. Consumer Health Data does not include information that is publicly available, de-identified, or aggregated. What Consumer Health Data we collect may differ depending on your relationship with us.

Our Purposes for Collecting and Using Consumer Health Data
We collect, use, disclose, or otherwise process your Consumer Health Data as necessary to provide our Services to you (specifically, to assess your risk of falling within a particular premises and to recommend related accommodations to such premises) or to correspond with you in connection with our Services. We may also process your Consumer Health Data as described in the Other Permitted Purposes for Processing Personal Data section of our Privacy Policy.
We do not engage in geofencing activities of in-person health care facilities for any purpose.

If you are a Washington resident, you have rights set forth in this section. Please note that your rights may be subject to certain conditions and exceptions under applicable law.
To exercise your rights, you must send us a request that (1) provides sufficient information to allow us to verify that you are the person about whom we have collected Personal Data (such as your login credentials, name and/or address, as applicable), and (2) describes your request in sufficient detail to allow us to understand, evaluate and respond to it. Each request that meets both of these criteria will be considered a “Valid Request.” We may not respond to requests that do not meet these criteria.
